Living Instruments is inviting engineering societies, safety-professional associations, and standards organizations to help develop and review the Safety Reporting Standards program.

The reporting journey

  1. Report. Preserve the concern in the reporter’s own words.
  2. Acknowledge. Confirm what was received and who is responsible.
  3. Assess. Consider supporting and contrary evidence.
  4. Act. Record the decision, reasons, and proportionate response.
  5. Verify. Check what the action actually changed.
  6. Return. Give the reporter the outcome, unresolved issues, and review route.

Participation and review

The program’s draft calls for disclosed conflicts, clear evidence requirements, reasoned decisions, correction, and appeals. Standards are in development; no accreditation or certification is claimed.
